Sliding Fee Schedule
Sprout Dental is proud to be a National Health Service Corps care provider. This allows us to better serve our hometown and surrounding rural communities by offering services at discounted rates, and sometimes for free, (using a sliding fee schedule) to low-income, uninsured patients.
The sliding fee schedule, or discounted fee schedule, is based upon the current Federal Poverty Guidelines, and patient eligibility is determined by annual income and family size.
To determine eligibility, we may require verification in the form of tax returns and/or current pay stubs. We also base evaluations on current participation in certain federal/state public assistance programs including Social Security Disability Income (SSDI), Temporary Assistance for Needy Families (TANF), Free or reduced lunch program, and other public assistance programs. Please consider bringing any of these appropriate forms to your appointment should you want to participate in this program.
Also, please know, that Sliding Fee Applications must be submitted annually to account for any changes to insurance, employment, and income.
